REQUIREMENTS TO PLAY

• We welcome all people who are committed to having a good time and support our Mission.
• Players must be at least 18 years of age or older
• Must have a photo identification to play (only for verification on waiver)
• Must complete and sign forms below before playing.  We usually do this at the field but we ask you to complete the form at the bottom of this page so it’ll be pre-printed for you.
  (this includes referees, coaches, photographers, etc.)
• We welcome all people who are committed to having a good time and abiding by our mission statement.

WAIVER & INDEMNIFICATION

This waiver is provided for your review.  You will be required to produce a valid government or school-issued identification with your photo on it when you come to the field for the first time to sign this document.  Should you have any inquiries, please utilize the “Contact Us” Form.
